About the office
The following information about some of our office policies will help you to become better acquainted with the way the office works.
Office Hours:
- Monday – Thursday: 9:00 a.m. to 5:00 p.m.
- Friday: 9:00 a.m. – 4.00 p.m.
Closed daily for lunch from 12:00 p.m. to 1:00 p.m.
Telephone calls to the doctor: Unless in an acute emergency you will not be able to talk to the doctor during office hours. Please leave your question with the office staff. Complex medical issues require an office visit or at the least, a telephone consultation at regular fees.
Emails to the doctor: Due to confidentiality issues, we can not correspond by email in regards to medical questions or concerns. If you have a medical question or concern please call our office staff. Complex medical issues require an office visit or at the least, a telephone consultation at regular fees.
Emergencies: In case of a medical emergency call 911 or go directly to the nearest emergency room.
Lab fees: The DaSilva Institute believes that a proactive approach to healthcare is the best medicine for prevention. There are several tests that your insurance carrier may not deem medically necessary and therefore will not reimburse. You may chose to use your insurance with Quest Diagnostics, although a limited “medically necessary” panel will be used. LabCorp is a ‘fee for service’, with tremendous discounted out of pocket fees. With LabCorp, we are not restricted as to what tests can be ordered. You must pay the DaSilva Institute directly for all labs ordered through LabCorp in order to receive our discounted rate. After the doctor puts you in for labs through LabCorp of America, you will be charged. A service charge of $35.00 will incur if you require a refund for any Labs already charge for.
Both LabCorp or America and Quest Diagnostics are nationwide laboratories. As such, you may perform testing anywhere in the United States.
If you choose to use your insurance for blood tests, we can order a limited panel of tests through Quest
Diagnostics; however, since there are several tests that your insurance carrier may not deem medically necessary, and therefore not reimbursable, we will not be able to order all of the tests necessary for prevention of disease (i.e., markers for breast cancer, insulin resistance, inflammation and bone loss).
We also use specialty laboratories such as Pharmasan Labs, Genova Diagnostics, Metametrix, and EnteroLab. For these Laboratories you mail the payment directly to them at the time of sending in the tests. All tests for heavy metals rendered by Doctors Data will be paid directly to the DaSilva Institute.
Lab results: We will only call you with the results of blood tests or other labs if they contain critical values that need immediate attention. Otherwise we will discuss lab results during your next doctor visit. Make sure to book a follow up appointment whenever you have blood drawn or any other lab procedure performed. You will not be able to get results without consulting with the doctor.
Consultations with the doctor: Consultations can either be in person or through telephone. You do not have to be in Florida to take advantage of the excellent diagnostic work-up and level of care you can experience at the DaSilva Institute. All blood work can be done in your own town (through LabCorp of America or Quest Diagnostics), and specialize testing can be performed in the comfort of your own home via tests kits we provide for you. We are currently serving patients throughout the world.
Request for medical records: Each time you see the doctor you will be provided with a printed or a digital copy of your lab records. If additional records are requested by you, or by an authorized third party such as another doctor’s office or insurance provider, you will be required to pay the DaSilva Institute a minimum fee of $20 plus a $0.50 per page fee for reproduction of any part of your chart requested (only if authorized by the patient).
Changing or canceling appointments: Please let us know as early as possible if you need to change your doctor appointment. We regret we must charge for missed or changed appointments, unless we are given one full business day’s notice. (Business days are Monday to Friday). For changes within one full business day you will be responsible for the full service fee.
Renewal of medications: Unless you schedule a follow-up visit every six months, we cannot fill your prescription. Renewal of medication should be done well in advance. We do not refill medication after office hours when your medical record is unavailable for review. To get refills please have your pharmacy fax us a refill request at (941) 388-0921.
Supplement orders: You can either order directly from this website or you can reorder your supplements by calling our office at 941-388-0940, or emailing your orders to orders@DaSilvaInstitute.com.
All services must be paid in full at the time of service.
We accept Visa, Master card, Debit cards with the Visa or MasterCard logo, checks- both personal and business (with proper ID) and cash for all services and products.
We do not accept any form of insurance. However we do provide a receipt with all information so that you can bill your own insurance for possible reimbursement.
Limitation of Services:
The DaSilva institute will not replace your existing primary care physician. It is your responsibility to continually get your basic medical checkups by a qualified Internist, Family Physician, or OB/GYN. The DaSilva Institute will act solely as a Specialist for your Antiaging, Regenerative and Functional Medicine needs, in accordance to what you came in for (i.e., hormone replacement will only deal with hormonal issues, gastrointestinal issues with GI treatments, and so on).
